Nursing Care
Registered nurses (RNs) provide medical care such as administering medications, managing wounds, monitoring vital signs, and offering health education to patients and families. They also assist in chronic disease management and post-operative care.
Physical Therapy (PT)

When you and your doctor decide that home health care is right for you, Ruby’s Home Health will take the lead in making the process seamless and comfortable.
Assessment at Your Home:
One of our team members will visit your home to assess your situation and medical condition(s). During this visit, we will develop a personalized plan of care tailored to your specific needs.
Your Care Plan
This plan may include services such as skilled nursing, therapies, equipment, and other treatments. The plan will be reviewed regularly by our team in collaboration with your doctor to ensure it remains effective.
Ongoing Visits
Our team will make regular visits to your home to provide care and monitor progress. We will also educate you and your caregivers on managing treatments such as wound care, therapy, and chronic disease management.
Adjustments to Your Care
If your condition changes, we will coordinate with you and your doctor to modify the plan of care as needed, ensuring you always receive appropriate and effective support.
Discharge and Next Steps
Once your goals are met, your doctor will determine if home health services are no longer needed. If additional care is required, we will help transition you to outpatient facilities or follow-up appointments, as recommended.
